Recreational Water Quality Illness Survey
White River Waterkeeper (WRW) is conducting a survey related to illnesses associated, or thought to be associated, with recreational surface waters (i.e., rivers, lakes, streams, springs). Participation in this survey will help further the understanding of environmental health concerns and identify where additional resources are needed for water quality monitoring and how best to prevent widespread illness rates.
Results may be summarized and published, omitting any personal or identifying information offered by participants. If additional information is identified as necessary after this survey, WRW may contact you requesting participation in a follow-up survey. If we are contacted by local, state, or federal healthcare professionals requesting personal information, we will forward contact information of the requesting party and instructions on how you can provide this information personally and willingly.
Besides email address, responses to all questions are optional. WRW will not use your email address or other contact information for any purposes besides following up with individuals surveys if necessary. To stay informed of survey results, findings, and next steps, you must opt below to sign up for notifications from WRW.
Survey will take approximately 10-15 minutes to complete. You will be able to request a copy of your responses at the end of this survey and will have the option of editing results after it is completed. If you do not feel comfortable with how we plan on using the survey results, please do not participate.
What’s the reason for this survey?
Following multiple reports of illnesses arising after swimming in, or accidentally ingesting, water from rivers within the watershed, White River Waterkeeper has developed a Recreational Water Quality Survey to better understand any trends and better identify concerns and solutions.
